EMPLOYEE RESPONSIBILITIES
- Report the work-related injury or illness to your supervisor at the time of injury or as soon as possible.
- Report to the Workers' Comp Coordinator’s office to complete all necessary forms documenting the injury and obtain an Authorization for Treatment form if treatment beyond first aid is required.
- If immediate medical attention is required, seek care first, then notify your supervisor and the Workers' Comp Coordinator.
- Provide detailed information about the scene, contributing conditions, and witness contact information.
- Follow all employer reporting procedures and complete required documentation.
- Cooperate with any employer-led investigations regarding the incident.
SUPERVISOR RESPONSIBILITIES
- Assist the employee in immediately reporting the work-related injury or illness.
- If the injury or illness is severe, call 911 and notify Campus Police at (504) 520-7490 immediately.
- Encourage employees to report any potentially hazardous work environments.
- Ensure employees wear required personal protective equipment (PPE), maintain a hazard-free environment, and ensure unsafe acts or practices are never tolerated.
WORKPLACE SAFETY CLAIMS COORDINATOR RESPONSIBILITIES
- Complete the 1st Injury Report, notify the workers' compensation insurance carrier, and obtain a claim number.
- Refer the injured employee for appropriate medical care.
- Report any unsafe working environments to the Director of Environmental Health & Safety.
- Ensure compliance with occupational injury and illness recordkeeping and reporting guidelines set by federal and state agencies, including OSHA.
